The Pre-T-cell receptor alpha subunit (PTCRA), also known as pTalpha, is a type I transmembrane glycoprotein that serves as a surrogate for the TCR-alpha chain during early T-cell development (UniProt: Q6ISU1). It forms the pre-T-cell receptor (pre-TCR) complex by pairing with a rearranged TCR-beta chain and CD3 signaling molecules (PubMed: 10880535). This complex is vital for the beta-selection checkpoint in the thymus, signaling for the survival, proliferation, and further differentiation of double-negative thymocytes into double-positive cells (PubMed: 15662446). Beyond its physiological role, PTCRA is often aberrantly expressed or dysregulated in T-cell acute lymphoblastic leukemia (T-ALL), where it promotes leukemic cell survival and expansion (PubMed: 22491048). Due to its restricted expression on early T-cell precursors and leukemic blasts, it is considered a promising target for immunotherapy, including monoclonal antibodies and CAR-T cell approaches (PubMed: 30206134). Therapeutic intervention targeting PTCRA aims to eliminate malignant cells while minimizing damage to the mature T-cell repertoire, though it may temporarily impair new T-cell production.
