Overview

The pro-inflammatory cytokine and inflammasome pathways are essential components of the innate immune system that mediate the body's response to pathogens and cellular stress (PubMed: 31036931). At the core of these pathways is the inflammasome, a multi-protein complex—most notably the NLRP3 inflammasome—that facilitates the activation of the protease Caspase-1 (UniProt: Q96P20). Activated Caspase-1 subsequently cleaves the inactive precursors of Interleukin-1 beta (IL-1β) and Interleukin-18 (IL-18) into their potent, biologically active forms (PubMed: 27951881). These cytokines are major drivers of systemic inflammation and can trigger pyroptosis, a highly inflammatory form of programmed cell death (PubMed: 30111834). Chronic or excessive activation of these pathways is a hallmark of numerous diseases, including autoinflammatory syndromes, gout, atherosclerosis, and neurodegenerative conditions like Alzheimer's disease (PubMed: 29385115). Therapeutic intervention typically involves the use of monoclonal antibodies to neutralize cytokines, receptor antagonists to block signaling, or small molecules to inhibit inflammasome assembly (Nature Reviews Drug Discovery: 10.1038/nrd.2017.243). Drugs such as canakinumab and anakinra have successfully targeted these pathways to treat conditions like rheumatoid arthritis and cryopyrin-associated periodic syndromes (FDA Label: Ilaris, Kineret). However, because these pathways are vital for host defense, therapeutic modulation carries risks such as an increased susceptibility to opportunistic infections (StatPearls: IL-1 Inhibitors).

Mechanism of action

The primary mechanisms involve the inhibition of the NLRP3 inflammasome assembly to prevent Caspase-1 activation, the direct neutralization of circulating pro-inflammatory cytokines like IL-1β using monoclonal antibodies, or the competitive antagonism of cytokine receptors such as IL-1R to block downstream signal transduction (PubMed: 31036931, Nature Reviews Drug Discovery: 10.1038/nrd.2017.243).
