The pro-inflammatory cytokine milieu refers to a complex microenvironment characterized by an elevated concentration of signaling proteins that promote systemic or local inflammation (Dinarello, 2018). This milieu typically includes key mediators such as tumor necrosis factor-alpha (TNF-α), interleukin-1 (IL-1), interleukin-6 (IL-6), and various chemokines (Zhang & An, 2007). These molecules are primarily secreted by immune cells like macrophages and T-lymphocytes to coordinate the body's response to injury or infection (Kopp & Medzhitov, 1999). However, a persistent or overactive pro-inflammatory milieu is a hallmark of chronic inflammatory diseases, autoimmune disorders, and cytokine release syndromes (Tanaka et al., 2014). Therapeutic strategies generally do not target the milieu as a whole but rather focus on inhibiting specific high-impact cytokines or their receptors to restore homeostatic balance (Feldmann & Maini, 2003). Because it represents a collective state of multiple distinct proteins rather than a single druggable entity, it is classified as a pathological environment rather than a discrete therapeutic target.
