Overview

Proinsulin autoantibodies (PAAs) are endogenous immunoglobulins that specifically target proinsulin, the biosynthetic precursor to insulin [1]. They are recognized as one of the earliest detectable biomarkers of pancreatic beta-cell autoimmunity, particularly in young children at risk for Type 1 Diabetes Mellitus (T1DM), where they often appear before clinical symptoms or other islet autoantibodies [2]. The presence of PAAs signifies a breakdown in immune tolerance toward islet antigens and indicates an ongoing immune-mediated destruction of the insulin-producing cells in the pancreas [1]. While PAAs themselves are not typically direct therapeutic targets, they are essential for patient stratification in clinical trials for disease-modifying therapies, such as Teplizumab, which aim to delay the progression of T1DM [3]. Monitoring PAA levels helps clinicians assess the risk of progression from stage 1 (autoimmunity) to stage 3 (clinical diagnosis) of the disease [2]. These antibodies are frequently measured alongside other markers like GAD65 and IA-2 autoantibodies to provide a comprehensive risk profile for individuals with a genetic predisposition to diabetes [1].
