Proinsulin-specific autoreactive CD8 T cells are a subset of cytotoxic T lymphocytes that recognize and respond to epitopes derived from proinsulin. These cells play a central role in the autoimmune destruction of pancreatic beta cells in type 1 diabetes (T1D). Their activity is considered a key effector mechanism leading to beta-cell loss and subsequent insulin deficiency. They infiltrate pancreatic islets, recognize proinsulin-derived peptides presented by HLA class I molecules on beta cells, and mediate their destruction. They are found within pancreatic islets and in the exocrine pancreas. Strategies aimed at reducing or modulating these specific autoreactive populations have shown promise for preserving residual beta-cell function.
