Proinsulin-specific T cell receptors (TCRs) are specialized membrane-bound proteins found on T lymphocytes that recognize and bind to peptide fragments derived from proinsulin, the precursor to insulin. These receptors are central to the pathogenesis of Type 1 Diabetes (T1D), as they mediate the selective autoimmune destruction of insulin-producing beta cells in the pancreatic islets (Mannering et al., 2010, Clinical & Experimental Immunology). In genetically susceptible individuals, autoreactive CD4+ and CD8+ T cells expressing these TCRs identify proinsulin epitopes presented by specific Human Leukocyte Antigen (HLA) molecules, such as HLA-DR4 or HLA-DQ8, triggering a chronic inflammatory response (Peakman, 2013, Diabetologia). Therapeutic strategies targeting these TCRs focus on antigen-specific immunotherapy, including peptide-based vaccines and TCR-engineered regulatory T cells (Tregs), which aim to reprogram the immune system toward tolerance (Lui et al., 2021, Frontiers in Immunology). By specifically addressing the T cell population responsible for beta cell damage, these treatments seek to preserve endogenous insulin production without the risks associated with broad-spectrum immunosuppression. Current clinical research continues to evaluate the efficacy of modulating these receptors to halt disease progression in newly diagnosed patients.
