Overview

The anticoagulant protein C/protein S pathway is a major endogenous system that regulates blood clotting and maintains vascular homeostasis (StatPearls, 2023). It is activated when thrombin binds to the endothelial receptor thrombomodulin, which then converts the zymogen protein C into activated protein C (APC) (UniProt, 2024; PubMed, 2003). APC, utilizing protein S as a non-enzymatic cofactor, proteolytically inactivates coagulation factors Va and VIIIa, thereby limiting further thrombin production (UniProt, 2024; PubMed, 2003). Beyond its role in anticoagulation, the pathway mediates significant anti-inflammatory, anti-apoptotic, and cytoprotective activities through the endothelial protein C receptor (EPCR) and protease-activated receptor-1 (PAR-1) (PubMed, 2003). Clinical deficiencies in protein C or protein S lead to a hypercoagulable state, manifesting as venous thromboembolism or neonatal purpura fulminans (NIH, 2020). Therapeutic strategies targeting this pathway include the administration of protein C concentrates for replacement therapy and the use of recombinant APC or thrombomodulin analogs to treat severe sepsis and thrombotic disorders (StatPearls, 2023; PubMed, 2003).

Mechanism of action

The pathway is initiated by the thrombin-thrombomodulin complex which activates protein C; activated protein C (APC) then works with protein S to inactivate Factors Va and VIIIa, while also signaling through EPCR and PAR-1 for cytoprotection (StatPearls, 2023; PubMed, 2003).
