Overview

Rapidly dividing tumor and normal proliferating cells refer to a biological state where cells are actively progressing through the cell cycle phases (G1, S, G2, and M) to facilitate growth and replication (NIH, 2023). This rapid proliferation is a hallmark of malignant progression, as cancer cells bypass normal regulatory checkpoints to achieve uncontrolled expansion and metastasis (National Cancer Institute, 2022). However, this phenotype is not unique to cancer; several essential healthy tissues also rely on rapid cell division for homeostasis, including hematopoietic stem cells in the bone marrow, the epithelial lining of the gastrointestinal tract, and hair follicles (StatPearls, 2023). Traditional cytotoxic chemotherapies exploit this high rate of division by targeting the molecular machinery required for DNA replication or mitotic spindle formation (PubMed, 2021). Because these therapeutic agents generally lack the ability to distinguish between malignant and healthy proliferating cells, they often result in significant systemic toxicities, such as neutropenia, anemia, and mucositis (American Cancer Society, 2023). Consequently, while these cells are the primary focus of many oncology treatments, the lack of molecular specificity remains a major challenge in clinical pharmacology.

Mechanism of action

Cytotoxic agents disrupt DNA synthesis, induce DNA damage, or interfere with microtubule assembly to trigger programmed cell death in cells actively transitioning through the cell cycle (StatPearls, 2023).
