Red blood cell (RBC) count is a fundamental clinical measurement that quantifies the number of erythrocytes per microliter of blood, serving as a primary indicator of an individual's oxygen-carrying capacity (StatPearls, 2023). Although 'Red blood cell count' is a physiological parameter rather than a molecular therapeutic target like a receptor or enzyme, it is a critical pharmacodynamic endpoint for therapies addressing hematological disorders (NIH, 2022). The production of red blood cells is primarily regulated by the hormone erythropoietin, which binds to the erythropoietin receptor in the bone marrow to stimulate erythropoiesis. Clinically, a low RBC count (anemia) may be caused by nutritional deficiencies, chronic kidney disease, or bone marrow failure, whereas an elevated count (polycythemia) can result from chronic hypoxia or myeloproliferative neoplasms (Mayo Clinic, 2023). Pharmacological interventions aim to modulate this count by either stimulating production using erythropoiesis-stimulating agents (ESAs) or by suppressing overproduction through cytoreductive therapies.
Drugs typically influence red blood cell count by acting as agonists for the erythropoietin receptor (EPOR) to stimulate erythropoiesis, providing essential substrates for hemoglobin synthesis (such as iron or Vitamin B12), or through cytoreduction to decrease excessive cell production in myeloproliferative disorders.
