Reduced diphtheria toxoid is a formaldehyde-inactivated form of the exotoxin produced by Corynebacterium diphtheriae, specifically formulated at a lower concentration for use in booster vaccinations (CDC, 2021). The "reduced" designation (often abbreviated as 'd' in Tdap) indicates a lower quantity of Limes flocculating (Lf) units compared to the standard pediatric DTaP vaccine, which helps minimize local adverse reactions in individuals who have already undergone primary immunization (FDA, 2020). Biologically, the toxoid acts as an immunogen that is processed by antigen-presenting cells to stimulate B-lymphocytes and T-helper cells, leading to the production of high-affinity IgG antibodies (DrugBank, 2023). These antibodies function by binding to and neutralizing the active diphtheria toxin, preventing it from entering host cells and inhibiting protein synthesis via ADP-ribosylation of elongation factor 2 (StatPearls, 2023). This neutralization is critical for preventing the severe clinical manifestations of diphtheria, including the formation of obstructive pseudomembranes in the airway and systemic damage to the myocardium and peripheral nerves (NIH, 2022). Consequently, reduced diphtheria toxoid is a vital component of public health strategies to maintain protective immunity against diphtheria in adolescent and adult populations.
