Regenerating islet-derived protein 3-gamma (REG3G) is a secreted C-type lectin with a carbohydrate recognition domain, primarily expressed in the intestine and other mucosal tissues[1][2][3]. REG3G binds to peptidoglycan on the surface of Gram-positive bacteria, exerting direct bactericidal activity to help maintain the mucosal barrier and modulate host-microbiota interactions[1][2][3][4]. It is induced in intestinal tissues after microbial colonization or infection and plays a protective role in mucosal immunity. Beyond its antimicrobial action, REG3G is involved in regulation of inflammation and tissue regeneration, with evidence pointing to roles in diseases such as pancreatitis, inflammatory bowel conditions, certain infections, and possibly some cancers and inflammatory heart conditions[1][3]. No approved drugs directly target REG3G, but expression is altered in response to various inflammatory and microbiota-altering interventions[3].
