Overview

Regulated endocrine-specific protein 18 (RESP18) is a luminal protein localized mainly to the dense core vesicles of pancreatic islet cells and neuroendocrine tissues, where it participates in secretory processes such as granule cargo packing and regulated secretion[1][3][4][6]. Its expression is induced by high glucose and is genetically linked with other dense core vesicle proteins (e.g., IA-2)[1][6]. RESP18 shows differential expression in response to neurotoxic stress and may contribute to ER stress and neuronal cell death in specific models[1]. Mutations in the RESP18 gene have been implicated in hereditary non-syndromic cleft lip and palate, suggesting a critical developmental role[2]. While RESP18 is biologically relevant, it is not currently considered a mainstream therapeutic target, and no drugs are known to target it directly.
