Regulatory T cells (Treg, commonly CD4⁺CD25⁺FOXP3⁺ cells) are a specialized T lymphocyte subset crucial for maintaining immune homeostasis and self-tolerance by actively suppressing excessive or aberrant immune responses, thereby preventing autoimmunity, controlling inflammation, and maintaining tolerance to self-antigens and transplants[3][10]. The Treg pathway involves numerous molecular and cellular processes regulating their development (driven by FOXP3 expression), expansion, trafficking, suppressive mechanisms (including cytokine release, cell-cell contact, and metabolic disruption), and functional plasticity. Treg-based therapies—including adoptive cell transfer and drugs modulating Treg abundance or function—are in various stages of clinical development for immunological diseases (autoimmunity, transplantation) and cancer, but therapeutic targeting poses challenges regarding specificity, stability, and risk of immune imbalances[2][4]. Note: “Regulatory T cells pathway” is not a molecular target but describes the pathway and function of a cell type; it cannot be mapped to a canonical drug target entity without additional specification (e.g., "FOXP3," "CD25," "GARP" as possible target molecules within this pathway)[2][4][10].
Mechanism of action for drugs targeting the Treg pathway involves enhancement of Treg function or number (leading to immune suppression), inhibition of Treg suppression (for anti-tumor immunity), depletion of Treg in the tumor microenvironment (for cancer immunotherapy), and adoptive cell transfer of Treg to promote immune tolerance.
