Rehmannia glutinosa, commonly known as Chinese Foxglove or Di Huang, is a fundamental perennial herb in Traditional Chinese Medicine (TCM) that has been used for centuries to 'nourish Yin' and 'tonify the blood.' It is not a single molecular target or receptor but rather a complex botanical source containing over 70 bioactive compounds, including iridoid glycosides (notably catalpol), phenylethanoid glycosides (such as acteoside), and various polysaccharides [7, 9]. These constituents act on a diverse array of molecular targets including AKT1, TNF-alpha, and NF-kappaB, which accounts for the plant's broad pharmacological profile [1, 5]. Research has highlighted its potential in managing type 2 diabetes by improving insulin resistance and its role in treating neurodegenerative and cardiovascular diseases through potent antioxidant and anti-inflammatory properties [4, 10]. Clinically, Rehmannia glutinosa extracts are employed to support renal health, promote blood cell production in anemic conditions, and mitigate symptoms of menopause [5, 11]. While generally considered safe in traditional practice, its multi-component nature necessitates careful monitoring for potential drug-herb interactions, particularly with glucose-lowering and blood-thinning therapies [6, 8].
Rehmannia glutinosa exerts its therapeutic effects through a multi-target mechanism involving its primary bioactive constituents, such as catalpol and acteoside. It modulates metabolic pathways by activating the PI3K/AKT signaling pathway to enhance insulin sensitivity and glucose uptake [10, 13]. Additionally, it provides anti-inflammatory and cytoprotective effects by inhibiting the NF-kappaB and MAPK signaling pathways, thereby reducing the production of pro-inflammatory cytokines like TNF-alpha and IL-6 [1, 5]. It also promotes hematopoiesis by stimulating the proliferation and differentiation of hematopoietic stem cells through the modulation of the bone marrow microenvironment [8, 13].
