Overview

Renal autonomic nerve fibers comprise both afferent (sensory) and efferent (sympathetic) fibers that innervate the kidney, primarily surrounding the renal artery and its branches and affecting renal blood vessels, tubules, and juxtaglomerular cells[1][5]. Most are unmyelinated sympathetic fibers that regulate renal blood flow, promote sodium and water reabsorption, and control renin release, playing essential roles in physiological homeostasis and the pathogenesis of hypertension and kidney disease[1][3][4][5]. The fibers work through norepinephrine-mediated activation of adrenergic receptors (mainly α-receptors), with little to no evidence of direct parasympathetic (vagal) innervation in the kidney[5]. Pathological overactivity or dysregulation of these nerves is strongly implicated in hypertension, chronic kidney disease, and cardiovascular outcomes, forming the rationale for therapeutic interventions such as renal denervation and pharmacological autonomic modulators[2][3][4][6].
