Overview

Renal blood flow (RBF) is the volume of blood delivered to the kidneys per unit of time, representing approximately 20% to 25% of the total cardiac output in a healthy adult (StatPearls, 2023). It is a fundamental physiological parameter rather than a single molecular target, serving as the primary determinant of the glomerular filtration rate (GFR) and the delivery of oxygen and nutrients required for renal metabolic processes (NIH, 2022). RBF is tightly regulated by intrinsic autoregulatory mechanisms, such as the myogenic response and tubuloglomerular feedback, as well as extrinsic factors including the sympathetic nervous system and the renin-angiotensin-aldosterone system (RAAS) (PubMed, 2021). Pharmacological agents often modulate RBF by targeting the molecular pathways that control renal vascular resistance; for instance, ACE inhibitors and ARBs promote efferent arteriolar dilation to manage hypertension and protect renal function (Merck Manual, 2023). Conversely, certain drugs like NSAIDs can dangerously reduce RBF by inhibiting vasodilatory prostaglandins, potentially leading to acute kidney injury in susceptible patients (PubMed, 2020). Maintaining adequate RBF is essential in clinical settings to prevent ischemic damage and manage chronic conditions like heart failure and chronic kidney disease.
