Overview

The renal proximal tubule organic anion transporters (OATs) are a group of membrane proteins primarily responsible for the uptake and secretion of a wide variety of endogenous and exogenous organic anions in the kidney, particularly within the proximal tubules. These transporters play a crucial role in drug elimination, detoxification, and homeostasis by mediating the movement of substances from blood into urine. Key members include OAT1 (SLC22A6) and OAT3 (SLC22A8). They function as exchangers, mediating uptake of organic anions from blood into tubular cells in exchange for intracellular dicarboxylates. They handle a broad range of substrates including endogenous metabolites, drugs, toxins and uremic solutes. By facilitating excretion of drugs and metabolic waste products, these transporters help maintain systemic chemical balance and prevent accumulation of potentially toxic compounds. Alterations or inhibition/competition at these transporter sites can lead to drug-drug interactions affecting pharmacokinetics/toxicity and variability in therapeutic efficacy among individuals.
