Overview

Renal sympathetic nerve fibers are peripheral autonomic nerve fibers, primarily unmyelinated, arising from pre- and postganglionic sympathetic neurons in the spinal cord and sympathetic ganglia, that innervate the kidney to regulate blood flow, tubular reabsorption, and renin release. They comprise mainly efferent (sympathetic) fibers, but also afferent sensory fibers that participate in reflex control of cardiovascular function. In disease states such as hypertension and heart failure, overactivity of these fibers contributes to pathological increases in blood pressure and adverse kidney remodeling, making them a therapeutic target for renal denervation procedures. "Renal sympathetic nerve fibers" is not a single molecular entity (such as a receptor or protein), but a functionally diverse bundle of nerve fibers mediating autonomic control of renal physiology.

Mechanism of action

Ablation/reduction of nerve activity (renal denervation) lowers sympathetic drive to the kidney, reducing blood pressure and renin secretion. Pharmacologic modulation of adrenergic signaling indirectly affects renal sympathetic output.
