Overview

Residual tumor cells in the surgical cavity represent the microscopic or macroscopic malignant cells that persist at the site of primary tumor resection following surgery. These cells are the primary drivers of local recurrence and can serve as a reservoir for systemic metastasis, making their eradication a critical goal in oncological management (National Cancer Institute, 2023). While not a single molecular target, this cell population is the focus of various therapeutic interventions, including adjuvant systemic chemotherapy, localized drug delivery systems like carmustine-impregnated wafers, and intraoperative imaging agents designed to improve surgical margins (PubMed, PMID: 30135553). Effective management of the surgical cavity aims to achieve R0 resection, where no tumor cells are detectable at the margins, thereby significantly improving patient survival outcomes (StatPearls, 2023). Challenges in targeting these cells include the difficulty of detection at the microscopic level and the potential for therapeutic agents to interfere with normal wound healing processes.
