Overview

Resolving blood stasis (Huoxue Huayu) is a foundational therapeutic principle in Traditional Chinese Medicine (TCM) aimed at improving blood circulation and dissipating blood clots or stagnant blood. It is not a single molecular target but rather a pharmacological category describing the collective action of herbs and drugs that modulate hemostasis, rheology, and vascular tone (Li et al., 2018; PubMed PMID: 29503591). Modern pharmacological research identifies its effects as involving anticoagulation, anti-platelet aggregation, fibrinolysis, and the improvement of microcirculation by acting on multiple pathways such as the arachidonic acid cascade and thrombin activity (Wang et al., 2017; PubMed PMID: 28435134). Clinical applications for resolving blood stasis include the treatment of coronary heart disease, stroke, and chronic inflammatory conditions where blood flow is compromised. Agents that fulfill this role, such as Salvia miltiorrhiza or standardized extracts like hirudin, interact with various proteins in the coagulation system and the vascular endothelium to reduce blood viscosity (Chen et al., 2016). Because it encompasses a broad range of biological processes and multiple molecular players, it is considered a therapeutic concept or functional effect rather than a discrete biological target like a receptor or enzyme.

Mechanism of action

Resolving blood stasis is a therapeutic principle that acts through multi-target mechanisms including the inhibition of the coagulation cascade (e.g., thrombin inhibition), modulation of the arachidonic acid pathway to inhibit platelet aggregation (e.g., TXA2/PGI2 balance), and the enhancement of endothelial nitric oxide (NO) production to promote vasodilation.
