Overview

Retinal bisretinoid lipofuscin is a complex mixture of fluorescent pigments that accumulate within the lysosomes of retinal pigment epithelium (RPE) cells as a byproduct of the visual cycle. The primary and most studied component is A2E (N-retinylidene-N-retinylethanolamine), which is formed through the non-enzymatic reaction of two molecules of all-trans-retinal with phosphatidylethanolamine in photoreceptor outer segments. Over time, the progressive accumulation of these bisretinoids becomes cytotoxic, inducing oxidative stress, lysosomal membrane permeabilization, and complement activation, which ultimately leads to RPE cell death and secondary photoreceptor loss. This pathological process is a hallmark of Stargardt disease, an inherited juvenile macular degeneration, and contributes significantly to the progression of geographic atrophy in age-related macular degeneration (AMD). Therapeutic strategies targeting this molecule focus on reducing its formation by modulating the visual cycle or using deuterated vitamin A to slow the rate-limiting dimerization step. Additionally, research into lipofuscinolytic agents aims to clear existing deposits from the RPE to restore cellular function and prevent further vision loss.
