Overview

The retrievable inferior vena cava (IVC) filter is a medical device implanted into the IVC, a large abdominal vein, to capture blood clots and prevent pulmonary embolism (PE) in patients at high risk for venous thromboembolism (VTE) who cannot use anticoagulation.[1][3][4] These filters, unlike permanent ones, feature a hook or retrieval mechanism allowing removal via a percutaneous procedure, typically from jugular or femoral access using snares, sheaths, or forceps once PE risk decreases.[1][2][6][7] Common designs include conical types like Günther Tulip (Cook Medical), Celect Platinum (Cook), Denali (Bard), and Option Elite (Argon), made from materials such as nitinol or cobalt-chromium, deployable via small sheaths (5F-9F) into IVCs up to 32 mm diameter.[3][5] Retrieval success rates exceed 90-99% in studies, though challenges arise with prolonged dwell times (>6 months), including filter tilt, strut penetration, thrombus entrapment, endothelialization, or embedding, sometimes requiring advanced techniques like loop snares or dual access.[2][3] Guidelines recommend retrieval when anticoagulation resumes safely, with low PE risk, and no ongoing filter indication, balancing in situ risks like caval thrombosis or fracture against procedural complications.[1] These devices expand treatment options for prophylactic or temporary VTE protection but are not therapeutic molecular targets.[1][2][3]
