Overview

Rheumatoid arthritis fibroblast-like synoviocyte (RA-FLS) proliferation is the primary effector process responsible for the aggressive synovial hyperplasia and joint destruction characteristic of rheumatoid arthritis (PMID: 30116111). In the RA joint, these cells transition to a tumor-like state, exhibiting rapid proliferation, reduced apoptosis, and enhanced migratory and invasive properties (PMID: 28814444). This proliferation is driven by an autocrine and paracrine loop of inflammatory cytokines, such as TNF-alpha, IL-6, and IL-1 beta, which activate intracellular signaling pathways including JAK/STAT, MAPK/ERK, and PI3K/Akt (NIH: PMC4045293). While RA-FLS cell proliferation is a critical pathological process and a major focus for drug development, it is a cellular phenotype rather than a single molecular target. Therapeutic strategies aimed at this process involve the use of DMARDs, JAK inhibitors, and biologic agents that target the specific receptors and enzymes regulating synoviocyte growth (PubMed: 31553487). Consequently, monitoring and inhibiting RA-FLS proliferation is essential for preventing the formation of the invasive pannus and subsequent cartilage and bone erosion.
