Overview

Saliva substitute refers to a range of over-the-counter artificial preparations designed to temporarily relieve symptoms of dry mouth (xerostomia) by mimicking the lubricating and protective properties of natural saliva. These substitutes typically contain water, cellulose derivatives or thickeners (e.g., carboxymethylcellulose, sorbitol, glycerol), sometimes with added electrolytes, flavoring agents, or pH buffers. Unlike biological targets, saliva substitute is a symptomatic replacement that does not modulate endogenous pathways or molecular targets; it acts locally by wetting and protecting the oral mucosa. Artificial saliva is offered in forms such as sprays, lozenges, gels, rinses, and mouthwashes. There is no universal equivalent to the complex mixture of proteins, mucins, and antimicrobial peptides found in natural saliva. These products are mainly intended for patients who cannot use salivary stimulants or whose salivary gland function is irreversibly impaired.

Mechanism of action

Forms a protective lubricating film over oral tissues to temporarily replace or supplement diminished saliva

03

Biological functions

Moisturization of oral tissueLubrication to facilitate speaking, chewing, and swallowingTemporary mechanical protection of the oral mucosa
04

Disease associations

Alleviation of xerostomia (dry mouth), which can be secondary to radiation, medications, Sjögren syndrome, diabetes, cancer therapy, and other conditions
05

Safety considerations

Generally well-tolerated. Rare risk of allergic reactions to excipientsEffectiveness and mouth-feel may vary between formulations; artificial saliva does not stimulate natural saliva production and does not replace all complex natural saliva functions
