Overview

Salivary proteins constitute a diverse and complex mixture of over 3,000 proteins and peptides secreted by the major and minor salivary glands into the oral cavity. This proteome includes enzymes like alpha-amylase for carbohydrate digestion, mucins for lubrication and mucosal protection, and antimicrobial agents such as histatins, lysozyme, and secretory IgA that maintain oral health. Beyond their local roles in the mouth, salivary proteins are increasingly recognized as a 'liquid biopsy' medium, reflecting systemic physiological states and serving as non-invasive biomarkers for various conditions, including stress, neurodegenerative diseases, and cancers. While the collective group is not a single therapeutic target, specific components like salivary alpha-amylase are targeted by inhibitors to manage postprandial glucose, and histatin-derived peptides are explored as novel antifungal treatments. Furthermore, the modulation of salivary protein secretion is a primary goal in treating xerostomia (dry mouth) using secretagogues that act on glandular receptors.

Mechanism of action

Specific salivary proteins like alpha-amylase are inhibited to slow starch breakdown, while others like histatins are used as antimicrobial agents to disrupt fungal membranes. Systemic drugs often interact with the production or composition of these proteins, such as cyclosporine-induced changes in salivary cytokines that contribute to gingival overgrowth.
