The saphenous nerve is the longest and largest sensory-only branch of the femoral nerve, originating from the L3 and L4 spinal nerve roots (StatPearls). It provides cutaneous innervation to the medial aspect of the lower leg, extending from the knee down to the medial malleolus and the arch of the foot (NIH). In clinical practice, the saphenous nerve serves as a primary anatomical target for regional anesthesia and analgesia, particularly for orthopedic procedures such as total knee arthroplasty or ankle surgery (PubMed). While the nerve itself is an anatomical structure rather than a single molecule, it contains voltage-gated sodium channels that serve as the molecular targets for local anesthetics like bupivacaine or ropivacaine (StatPearls). Dysfunctions of the saphenous nerve, such as entrapment in the adductor canal or injury during surgery, can lead to neuropathy characterized by chronic pain or paresthesia in the medial leg (Wikipedia). Management typically involves pharmacological blockade, neurolysis, or surgical decompression to restore function or alleviate symptoms (StatPearls).

Mechanism of action

Local anesthetics target the saphenous nerve by binding to voltage-gated sodium channels on its axons, which inhibits sodium influx and prevents the initiation and propagation of action potentials, thereby blocking sensory pain transmission (StatPearls).
