Overview

SARS-CoV-2 spike protein-specific CD4+ T cells are a specialized subset of helper T lymphocytes that recognize peptide fragments of the viral spike protein presented by Major Histocompatibility Complex (MHC) class II molecules (Grifoni et al., 2020, Cell). These cells are pivotal for a coordinated immune response, as they provide essential signals for B cell maturation and antibody isotype switching, as well as support for the expansion of cytotoxic CD8+ T cells (Sette & Crotty, 2021, Cell). In the context of COVID-19, the presence of these cells is strongly correlated with protective immunity and milder disease outcomes (Dan et al., 2021, Science). They are the primary cellular targets of most COVID-19 vaccines, such as BNT162b2 and mRNA-1273, which aim to induce robust and long-lasting T cell memory (Sahin et al., 2020, Nature). Monitoring these cells via markers like IFN-gamma and CD154 serves as a critical measure of vaccine efficacy and the durability of the immune response against emerging viral variants (Painter et al., 2021, Science Immunology).
