Overview

The sciatic nerve is the largest and longest single nerve in the human body, formed by the union of spinal nerve roots L4 through S3 of the lumbosacral plexus. It exits the pelvis through the greater sciatic foramen and descends through the gluteal region into the posterior thigh, where it eventually bifurcates into the tibial and common fibular nerves. As a mixed peripheral nerve, it provides motor innervation to the hamstring muscles and nearly all muscles of the leg and foot, while also transmitting sensory information from the skin of the lower limb. In a clinical context, the sciatic nerve is frequently the site of pathology rather than a specific molecular target; conditions such as sciatica involve the compression or inflammation of this nerve, leading to radiating pain and motor deficits. Pharmacological management often involves the use of local anesthetics for regional blocks, gabapentinoids for neuropathic pain modulation, and anti-inflammatory agents to reduce nerve root irritation. While it contains many specific molecular targets like sodium channels and calcium channel subunits, the nerve itself is categorized as an anatomical structure.

Mechanism of action

Drugs affecting the sciatic nerve typically act by blocking voltage-gated sodium channels to inhibit nerve conduction (local anesthetics), modulating the alpha-2-delta subunit of voltage-gated calcium channels to reduce excitatory neurotransmitter release (gabapentinoids), or inhibiting cyclooxygenase (COX) enzymes and reducing inflammatory cytokines to alleviate compression-related inflammation (NSAIDs and corticosteroids).
