Overview

Sebum is a complex mixture of lipids produced by the sebaceous glands, primarily composed of triglycerides, wax esters, squalene, and free fatty acids (StatPearls, 2023). It plays a vital role in human physiology by maintaining the epidermal permeability barrier, providing antimicrobial protection, and transporting fat-soluble antioxidants to the skin surface (PubMed, 2017). However, the dysregulation of sebum production is a central factor in several dermatological conditions, most notably acne vulgaris, where excessive sebum (seborrhea) facilitates the growth of Cutibacterium acnes and contributes to follicular occlusion (NIH, 2022). Hydrophobic surface debris consists of accumulated sebum combined with environmental pollutants and desquamated skin cells, which can impair skin hygiene and exacerbate inflammatory states. Therapeutic strategies do not target sebum as a receptor but rather aim to modulate its production through systemic retinoids like isotretinoin or topical anti-androgens like clascoterone (Journal of Clinical and Aesthetic Dermatology, 2020). Additionally, topical cleansers utilize surfactants to emulsify and remove these hydrophobic materials to maintain skin health and prevent comedone formation. While not a proteinaceous target, sebum levels are frequently used as a clinical endpoint in dermatological drug development.
