Sensory neurons in the dental pulp are specialized afferent nerve fibers, primarily A-delta and C-fibers, that originate from the trigeminal ganglion and provide innervation to the tooth's internal tissue (StatPearls, NBK542242). Their primary biological function is nociception, detecting potentially damaging stimuli such as extreme temperatures, high pressure, or chemical changes resulting from decay or injury (PubMed, 12002821). These neurons play a central role in the pathophysiology of pulpitis and dentin hypersensitivity, where inflammatory mediators sensitize the nerve endings, leading to intense pain (Journal of Dental Research, 10.1177/0022034510384621). While not a single molecular target, these neurons express a variety of therapeutic targets including voltage-gated sodium channels (Nav1.7, Nav1.8) and transient receptor potential (TRP) channels (Journal of Endodontics, 2018). Pharmacological management of dental pain involves the use of local anesthetics to block signal conduction or analgesics to reduce the activation threshold of these sensory units. In addition to pain transmission, these neurons release neuropeptides like Substance P and CGRP, which contribute to neurogenic inflammation and tissue repair processes within the pulp (PubMed, 15533517).
