Overview

The sensory neuropeptide release machinery refers to the specialized protein complex, primarily the SNARE (Soluble N-ethylmaleimide-sensitive factor Attachment Protein Receptor) complex, responsible for the regulated exocytosis of neurotransmitters and neuropeptides from sensory neurons. Key components include SNAP-25, syntaxin, and VAMP (synaptobrevin), which facilitate the fusion of synaptic vesicles with the plasma membrane in response to calcium influx. In sensory nociceptors, this machinery mediates the release of pro-inflammatory and pro-nociceptive peptides such as Calcitonin Gene-Related Peptide (CGRP) and Substance P, which are central to the pathogenesis of migraine and chronic pain states. Therapeutic intervention typically involves the use of botulinum neurotoxins, which act as zinc-dependent endopeptidases to cleave specific SNARE proteins, thereby paralyzing the secretory apparatus. By inhibiting the release of these neuropeptides, these drugs effectively reduce neurogenic inflammation and peripheral sensitization, providing relief for various neurological and pain disorders.
