SERPINB7 acts as an endogenous inhibitor of Lys-specific proteases and is implicated in maintaining epidermal homeostasis, regulating keratinocyte differentiation, and contributing to the integrity of the epidermal barrier[2][3]. Loss-of-function mutations lead to epidermal barrier defects and are causative for autosomal recessive Nagashima-type palmoplantar keratosis, with upregulation observed in psoriatic skin and mesangial expansion in kidney disease[2][3][7]. SERPINB7's biological and pathophysiological roles make it a candidate therapeutic target in inflammatory skin diseases and certain nephropathies, although no direct drugs, mechanisms, or major safety liabilities have been reported to date[2][3][9].
