Overview

SARS-CoV-2 IgM (Immunoglobulin M) is the primary antibody isotype produced by the host immune system during the early stages of infection with the Severe Acute Respiratory Syndrome Coronavirus 2 (Hou et al., 2020, Clinical & Translational Immunology). It typically becomes detectable in the blood within 3 to 7 days after the onset of symptoms, serving as a critical biomarker for identifying acute or recent COVID-19 infections (Long et al., 2020, Nature Medicine). Structurally, IgM exists as a pentameric complex, which provides high avidity for viral antigens such as the spike (S) protein and the nucleocapsid (N) protein, facilitating efficient neutralization and the activation of the classical complement pathway (Sterlin et al., 2021, Science Translational Medicine). While IgM plays a vital role in the initial defense against the virus, its levels are transient and generally decline as the more persistent and high-affinity IgG response matures. In the pharmaceutical and diagnostic industries, SARS-CoV-2 IgM is utilized for patient stratification, epidemiological surveillance, and monitoring the kinetics of the immune response rather than as a direct target for drug therapy (NIH, 2021).
