Overview

SH2B adapter protein 1 (SH2B1) is a key intracellular adapter protein that plays a critical role in regulating energy homeostasis and nutrient metabolism by enhancing signaling from various receptor tyrosine kinases. It acts as a powerful enhancer of leptin and insulin signaling pathways; specifically, it binds to Janus kinase 2 (JAK2) to promote leptin-mediated STAT3 activation and interacts with insulin receptor substrate (IRS) proteins to facilitate insulin action. Genetic deficiencies or mutations in the SH2B1 gene are strongly associated with severe early-onset obesity, insulin resistance, and hyperphagia in humans. While SH2B1 itself is difficult to target directly with small molecules due to its nature as an adapter protein, the mRNA and the resulting protein pathway are significant therapeutic focal points. Current clinical strategies involve using MC4R agonists like setmelanotide to bypass the signaling defects in the leptin-melanocortin pathway caused by SH2B1 insufficiency, and research into mRNA-based therapies or gene therapies to restore SH2B1 function remains an area of active investigation for metabolic disorders.
