The extract's primary component, silybin, works by protecting liver cells against oxidative stress and toxins while promoting the repair of damaged tissues through the stimulation of protein synthesis (PubChem, CID 31553). It is clinically utilized in treating conditions such as alcoholic liver disease, viral hepatitis, and mushroom poisoning (Mayo Clinic, 2023). Despite its long history of use, its status as a target is incorrect as it represents a multi-component drug product rather than a specific biological receptor or enzyme.

Mechanism of action

Milk thistle fruit extract acts primarily through its active complex, silymarin, which stabilizes hepatocyte membranes and scavenges free radicals (StatPearls, 2023). It stimulates RNA polymerase I in the nucleus, leading to increased ribosomal protein synthesis and enhanced liver regeneration (PubChem, CID 31553). Furthermore, it inhibits the NF-kappaB signaling pathway to reduce inflammation and suppresses TGF-beta1 to prevent fibrogenesis (NIH, 2021).
