The sinus mucosa, also known as the Schneiderian membrane, is the ciliated pseudostratified columnar epithelium that lines the paranasal sinuses (StatPearls, 2023). Its primary biological function is mucociliary clearance, a defense mechanism where goblet cells secrete mucus to trap inhaled particles and pathogens, which are then propelled by cilia toward the natural ostia (NIH, 2022). In diseases such as chronic rhinosinusitis (CRS) and nasal polyposis, the mucosa undergoes significant remodeling, including goblet cell hyperplasia and basement membrane thickening (JACI, 2020). While the sinus mucosa is the site of action for many drugs, it is an anatomical tissue rather than a specific molecular target like a receptor or enzyme. Pharmacological treatments, such as intranasal corticosteroids or monoclonal antibodies, interact with specific molecular pathways within this tissue to reduce inflammation and edema (PubMed, 2021). Consequently, while it is the focus of rhinology treatments, it represents a complex multicellular environment containing various therapeutic targets. Therapeutic interventions often aim to restore the barrier function and ciliary activity of the mucosa to prevent recurrent infections.
Pharmacological agents act on specific molecular targets within the sinus mucosa, such as glucocorticoid receptors to suppress inflammatory gene expression or cytokine receptors (e.g., IL-4Rα) to inhibit Type 2 inflammation and reduce tissue edema (StatPearls, 2023; PubMed, 2021).
