Overview

The skeletal system is a complex organ system composed of bones, cartilages, ligaments, and other connective tissues that provide the structural framework for the human body (StatPearls, 2023). Beyond its mechanical roles in support, protection of vital organs, and locomotion, it serves as a critical reservoir for minerals like calcium and phosphorus and houses the bone marrow responsible for hematopoiesis (StatPearls, 2023). In the context of pharmacology, the skeletal system is not a single molecular target but a physiological environment where various drugs act on specific cells—osteoblasts, osteoclasts, and osteocytes—to manage metabolic bone diseases (NIH, 2023). Conditions such as osteoporosis and Paget's disease are treated by modulating the balance between bone resorption and formation through pathways like RANK/RANKL/OPG and Wnt signaling (Nature Medicine, 2012). Understanding the skeletal system's dynamic remodeling process is essential for developing therapies that maintain bone integrity and prevent fractures (Clin Biochem, 2011).

Mechanism of action

Drugs acting on the skeletal system typically modulate bone remodeling by inhibiting osteoclast-mediated bone resorption (e.g., bisphosphonates, RANKL inhibitors) or stimulating osteoblast-mediated bone formation (e.g., PTH analogs, sclerostin inhibitors) (Nature Medicine, 2012; NIH, 2023).
