Skin surface pH refers to the measure of acidity or alkalinity on the outermost layer of human skin—primarily the stratum corneum. Healthy adult human skin typically maintains an acidic environment with a pH range between 4.0–6.0, often called the "acid mantle." This acidity is crucial for several biological functions including maintaining an effective permeability barrier against water loss and pathogens; supporting antimicrobial defense; regulating enzymatic activities involved in lipid processing; and ensuring proper cohesion within the stratum corneum. The acidification mechanisms involve breakdown products from filaggrin protein metabolism, free fatty acids from sebaceous secretions, sweat components, sodium-proton exchangers in keratinocytes, among others. The precise value varies depending on anatomical site (lower on trunk/extremities than intertriginous areas), age (higher at birth), ethnicity (slightly lower in darker-pigmented individuals), use of soaps/cosmetics (alkaline products raise it), presence of urine/feces on skin surfaces (raises it), among other factors. Disruption or elevation of normal skin surface pH has been implicated in various dermatological conditions such as atopic dermatitis—where higher-than-normal values are associated with impaired barrier recovery—and acne vulgaris. Therapeutic strategies often focus on restoring physiological acidity using mildly acidic cleansers/moisturizers to support optimal enzyme activity for ceramide synthesis and to suppress pathogenic microbial growth while promoting healthy commensal flora. Skin surface pH is not a molecular target for drugs but can be influenced by topical agents such as moisturizers, cleansers, and acidifying preparations. Not applicable as it is not a drug target. However, products that lower skin surface pH may restore barrier function and reduce inflammation by optimizing enzyme activity and inhibiting pathogenic microbes. Skin surface pH itself can serve as a biomarker for barrier integrity or disease state in dermatology. No direct safety concerns; however, altering skin surface pH with inappropriate products (e.g., alkaline soaps) can impair barrier function and increase risk of irritation or infection.
