Overview

Slow-wave sleep (SWS), also known as deep or NREM stage 3 sleep, is a physiological state characterized by high-amplitude, low-frequency delta oscillations (0.5–4 Hz) observed on an electroencephalogram (EEG). These slow waves reflect the synchronized rhythmic activity of thalamocortical networks alternating between hyperpolarized "down" states and depolarized "up" states. SWS is a critical target in modern drug development because it facilitates synaptic homeostasis and the glymphatic clearance of metabolic waste, including amyloid-beta and tau proteins, from the brain. Disruption of slow-wave activity is a hallmark of aging and is strongly associated with the pathogenesis of Alzheimer’s disease, Parkinson's disease, and major depressive disorder. Pharmacological strategies to enhance SWS involve the use of GABAergic agonists, such as sodium oxybate, or orexin receptor antagonists that stabilize sleep architecture and promote deep NREM sleep. However, therapeutic challenges include risks of excessive sedation, respiratory depression, and the potential for drug abuse or dependence.

Mechanism of action

Enhancement of slow-wave sleep (SWS) typically occurs through the modulation of GABAergic pathways (specifically GABA-B or GABA-A receptors) or the inhibition of wake-promoting systems (e.g., orexin receptor antagonism) to stabilize thalamocortical synchronization and promote deep NREM sleep.
