Smooth muscle restoration is a therapeutic objective and biological process focused on recovering the structural integrity and contractile function of smooth muscle tissues following injury, degeneration, or disease [4, 21]. It is not a single molecular target but rather a complex physiological outcome achieved through various regenerative strategies, including stem cell transplantation, growth factor delivery, and physical modalities like shockwave therapy [11, 21]. In the context of erectile dysfunction, it involves the regeneration of cavernous smooth muscle cells to restore the smooth muscle-to-collagen ratio necessary for proper blood trapping and erection [7, 20]. In vascular biology, it refers to the return of vascular smooth muscle cells from a synthetic or proliferative phenotype to a contractile phenotype [2, 19]. Key molecular drivers of this process include vascular endothelial growth factor (VEGF), endothelial nitric oxide synthase (eNOS), and transcription factors like myocardin [10, 21]. While not a single molecular target, smooth muscle restoration represents a critical endpoint for evaluating the efficacy of regenerative medicines in urology and vascular biology [5, 16].
