Overview

The **sodium-calcium exchanger** (NCX) is a plasma membrane antiporter protein that plays a crucial role in regulating intracellular calcium by exchanging three sodium ions for one calcium ion across the cell membrane. It is essential for maintaining low cytoplasmic Ca^2+ concentration, particularly after rapid calcium influx during electrical or contractile activity. NCX operates with low affinity but high capacity, making it effective for rapid Ca^2+ extrusion in excitable cells such as cardiac muscle, neurons, and photoreceptors. There are at least three main mammalian isoforms (NCX1 in the heart, NCX2 in the brain, NCX3 in skeletal muscle/brain), all sharing core transport and calcium-binding domains with subtle differences in regulation and tissue distribution. Dysfunction or inappropriate activation of NCX—for example, during ischemia—can contribute to cardiac arrhythmias, heart failure, or neuronal cell death. Pharmacological inhibitors such as SEA0400 block NCX activity and are being investigated as potential therapies for these pathologies, though safety concerns persist regarding interference with normal excitation-contraction coupling and neuronal signaling.
