Voltage-gated sodium channels NaV1.2 and NaV1.6, encoded by the SCN2A and SCN8A genes respectively, are essential for the initiation and propagation of action potentials in the central nervous system (NIH, 2024). These channels are primarily localized to the axon initial segment and nodes of Ranvier of excitatory pyramidal neurons, where they play a pivotal role in regulating neuronal excitability and signal transduction (BioRxiv, 2023). Pathogenic variants in these genes are linked to a broad spectrum of neurological disorders, including early-onset developmental and epileptic encephalopathies (DEE), autism spectrum disorder, and intellectual disability (Epilepsy.com, 2024). Gain-of-function mutations typically result in hyperexcitability and severe epilepsy, whereas loss-of-function mutations are often associated with neurodevelopmental delays (CHOP, 2024). While traditional anti-seizure medications like carbamazepine and phenytoin target these channels non-selectively, modern drug development focuses on isoform-selective inhibitors and genetic therapies to improve efficacy and safety by sparing the NaV1.1 isoform found in inhibitory interneurons (ACS, 2024).
