The Sodium voltage-gated channel alpha subunit 2 (SCN2A) gene regulatory region consists of the promoter and distal enhancers that orchestrate the expression of the Nav1.2 sodium channel, primarily in excitatory neurons during early postnatal development (Sanders et al., 2018, Neuron). Nav1.2 is essential for the initiation of action potentials at the axon initial segment, and its precise temporal and spatial expression is critical for normal brain function. Mutations affecting this gene lead to SCN2A-related disorders, where gain-of-function mutations cause severe early-onset epilepsies and loss-of-function mutations are a leading genetic cause of autism spectrum disorder and intellectual disability (Wolff et al., 2017, Brain). Therapeutic targeting of the SCN2A regulatory region aims to restore protein homeostasis; for instance, CRISPR-mediated activation (CRISPRa) can upregulate the healthy allele in haploinsufficiency cases by targeting the promoter, while antisense oligonucleotides (ASOs) like PRAX-222 are designed to reduce toxic gain-of-function expression (Praxis Precision Medicines, 2023). Because the brain is highly sensitive to Nav1.2 dosage, these therapies require precise calibration to avoid shifting a patient from one disease phenotype to its opposite.
