Overview

The soft tissue of the internal nasal valve (INV) refers to the mucosal and erectile tissues located at the narrowest portion of the human airway, bounded by the nasal septum, the caudal edge of the upper lateral cartilage, and the head of the inferior turbinate (StatPearls, 2023). This region is the primary site of nasal airflow resistance, where even minor changes in tissue volume significantly impact breathing according to Poiseuille's Law (PubMed, PMID: 31747208). While the soft tissue itself is an anatomical structure rather than a single molecular target, it contains high densities of alpha-1 and alpha-2 adrenergic receptors and glucocorticoid receptors that are targeted by decongestants and steroids (NIH, 2022). Pathological swelling of this tissue due to inflammation or structural collapse leads to symptomatic nasal obstruction, often requiring medical management or surgical interventions like functional rhinoplasty to restore patency (Wikipedia, 2024). Chronic use of topical agents on this tissue can lead to safety concerns such as rhinitis medicamentosa or mucosal damage (PubMed, PMID: 28291314).

Mechanism of action

Pharmacological agents typically target alpha-adrenergic receptors within the mucosa to induce vasoconstriction or glucocorticoid receptors to reduce inflammatory edema, thereby increasing the cross-sectional area of the nasal valve (StatPearls, 2023).
