Overview

Sporothrix is a genus of dimorphic fungi found globally in organic matter such as soil, moss, and decaying plants (CDC, 2020). The most clinically significant species, Sporothrix schenckii, is the primary causative agent of sporotrichosis, a subacute or chronic infection often referred to as rose gardener's disease (StatPearls, 2023). The organism is unique for its ability to transition from a saprophytic mold form in the environment to a parasitic yeast form upon entering host tissue at body temperature (NCBI, 2021). While most infections are localized to the skin and subcutaneous tissues following traumatic inoculation, disseminated forms can occur in immunocompromised individuals, affecting the lungs, joints, and central nervous system (NIH, 2022). Treatment typically involves prolonged antifungal therapy, with itraconazole serving as the first-line agent for cutaneous disease and amphotericin B reserved for severe systemic manifestations (WHO, 2022).

Mechanism of action

Antifungal drugs targeting Sporothrix species typically act by inhibiting ergosterol biosynthesis through the inhibition of lanosterol 14-alpha-demethylase (azoles) or squalene epoxidase (allylamines), or by binding directly to ergosterol to disrupt cell membrane integrity (polyenes) (StatPearls, 2023).
