Staphylococcus saprophyticus is a gram-positive, coagulase-negative, nonmotile coccus (0.8-1.2 µm diameter) that commonly causes community-acquired urinary tract infections (UTIs), particularly in young sexually active women. It colonizes the inguinal/perineal skin, adheres to uroepithelial cells via surface proteins, hemagglutinins, lipase, and biofilms, produces urease to promote growth and stone formation, and shows novobiocin resistance distinguishing it from similar staphylococci like S. epidermidis. Virulence involves biofilm production (protein-eDNA-polysaccharide matrix in most strains), lower adhesion proteins than other staphylococci, and heavy metal resistance; infections are treated with antibiotics like nitrofurantoin but can progress to pyelonephritis if untreated.
