Overview

Stool and intestinal mucosal surfaces refer to the anatomical and physiological environment of the gastrointestinal tract rather than a specific molecular target such as a receptor or enzyme. The intestinal mucosa is a multi-layered barrier composed of epithelial cells, a protective mucus layer, and underlying immune tissues that facilitate nutrient absorption while preventing the entry of pathogens (Frontiers in Immunology, 2023). Stool consists of undigested food, metabolic waste, and a vast community of microorganisms known as the gut microbiota, which plays a vital role in metabolic and immune health (Nature Reviews Microbiology, 2022). In clinical pharmacology, these surfaces are the site of action for locally delivered drugs, such as Mesalamine for ulcerative colitis or oral Vancomycin for Clostridioides difficile infections, which aim to achieve high local concentrations while minimizing systemic side effects (StatPearls, 2023). Because these are anatomical regions, they are not classified as traditional drug targets; however, their health is often assessed using biomarkers like fecal calprotectin to monitor inflammation (Journal of Gastroenterology, 2021). Maintaining the integrity of the mucosal surface and the balance of the stool microbiota is essential for preventing diseases like inflammatory bowel disease and colorectal cancer (NIH, 2024).
