Stool hydration via osmosis refers to the process by which water moves into or out of the intestinal lumen due to osmotic gradients created by retained solutes (such as undigested carbohydrates, ions, or drugs). This mechanism is central to normal stool formation and is the basis for the effect of osmotic laxatives, which increase stool water by retaining non-absorbable solutes in the bowel. It is not mediated by a single receptor or transporter, but involves the collective actions of epithelial ion channels (e.g., sodium and chloride transporters such as SGLT-1, NHE3, SLC26A3), aquaporins, and passive and active transport mechanisms. Dysregulation can result in diarrhea (excess water in stool due to poor solute absorption) or constipation (insufficient water in stool due to excessive absorption)[3][1][5][8].
