Overview

Stratum corneum corneocyte cohesion refers to the structural integrity and adhesion between the flattened, dead skin cells (corneocytes) in the outermost layer of the epidermis. This cohesion is maintained by a complex interplay of keratin intermediate filaments within the cells, specialized cell-cell junctions called corneodesmosomes, and a surrounding lipid-rich intercellular matrix. The strength of this cohesion is critical for the skin's barrier function, protecting against environmental insults and preventing excessive water loss. In various dermatological conditions, such as ichthyosis, psoriasis, and acne, this cohesion is pathologically increased, leading to hyperkeratosis and impaired desquamation. Therapeutic agents known as keratolytics, including alpha-hydroxy acids (AHAs), beta-hydroxy acids (BHAs), and urea, target this cohesion by disrupting the intercellular bonds or the protein structures holding the cells together, thereby promoting the shedding of dead skin cells and restoring a healthy skin surface.
