The stratum corneum extracellular lipid matrix is the specialized lipid-rich matrix found between corneocytes in the outermost layer of the epidermis (stratum corneum). It primarily comprises ceramides, cholesterol, and free fatty acids organized into highly ordered lamellar structures that form the main barrier to water loss and protect against environmental insults, including pathogens and irritants. The matrix is dynamic, with its structure and composition changing as cells move toward the skin surface. In addition to lipids, it contains minor but important proteins, enzymes, and antimicrobial peptides that help regulate barrier function and repair.
